Dear Primary Care Patients, 

We regret to inform you that Kevin Larochelle, FNP, will be leaving Cranmore Health Partners as of December 1st, 2024. Please plan to schedule an appointment with Kevin as soon as possible to begin an exit plan for your future primary care. 

Kevin will be returning to Coos County Family Medicine and will be able to see you at this practice if you desire to stay with him. It may take you months to establish care with another primary care provider so let us all be proactive and get you what is needed to make it to that appointment for your care. 

Please call the office and make an appointment with Kevin right away to establish this plan and get your records sent where needed. Cranmore Health Partners will not be replacing primary care at this time so once Kevin is gone you will not have access to Primary Care from this office.
